Team Care Arrangement — Chronic Disease Allied Health Referral (MBS Item 723, superseded by GPCCMP from 1 July 2025)
A coordinated care plan, formerly a separate Medicare form (MBS 723), that enabled GP referrals to up to five allied health providers for patients with chronic conditions; as of 1 July 2025 this is fully subsumed into the GP Chronic Condition Management Plan (GPCCMP) process, and a standalone TCA is no longer required.

How to apply
- Book a long (30+ minute) GP appointment and tell the receptionist you need allied health referrals for your chronic condition.
- Your GP confirms you have a condition present for 6 months or more (or terminal) and would benefit from multidisciplinary care.
- Your GP prepares a GP Chronic Condition Management Plan (GPCCMP, MBS item 965 or 392) — a standalone Team Care Arrangement (MBS 723) is no longer required as a separate document from 1 July 2025.
- Where allied health care is needed, the GP issues a standard referral letter directly to the relevant allied health provider (e.g. physiotherapist, dietitian, podiatrist).
- The allied health provider delivers services and claims Medicare rebates under the applicable individual allied health MBS items.
- Patients are entitled to up to 5 individual allied health services per calendar year under the chronic condition allied health framework.
- Reviews of the GPCCMP can occur every 3 months (MBS 967 / 393); patients should return to their GP as recommended.
- Patients who had a TCA in place before 1 July 2025 can continue accessing allied health services under that existing arrangement until 30 June 2027.
