How to apply

1. Confirm eligibility: donor-conceived adults (18+ or younger with counsellor approval), parents of donor-conceived children, donors, or descendants of donor-conceived people may apply.
2. Determine which register is relevant: Central Register (holds identifying information — names, dates of birth, donor codes) or Voluntary Register (for non-statutory connections and mutual contact).
3. Download and complete the application form from the VIC Department of Health (from 1 January 2025, VARTA's role transferred to the Donor Conception Registrar — email dcr@health.vic.gov.au for current forms); SA applicants use SA Health's Donor Conception Register; ACT applicants use the ACT Donor Register.
4. Submit certified proof of identity and pay the application fee ($84.60 per information category in VIC; fee waivers available for financial hardship).
5. Attend a free mandatory support session (~1 hour, in-person or remote) with the Registrar.
6. Complete a statement of reasons explaining your motivations and what outcomes you seek.
7. The Registrar conducts a register search and outreach to the other party (donor or donor-conceived person) to notify them and obtain their contact preferences.
8. Receive notification of outcomes; if both parties consent, information is exchanged via letters, email, or facilitated meetings.
