How to apply

1. Book a long (30+ minute) GP appointment and tell the receptionist you need a chronic disease management plan.
2. Your GP assesses whether you have at least one medical condition that has been or is likely to be present for 6 months or more, or is terminal.
3. If eligible, your GP prepares the GP Chronic Condition Management Plan (GPCCMP) — the current replacement for the old GP Management Plan (MBS 721, ceased 1 July 2025).
4. The plan documents your condition, treatment goals, actions, and identifies whether multidisciplinary care from allied health providers is needed.
5. If allied health referrals are required, your GP issues a standard referral letter (Team Care Arrangements are no longer required as a separate step).
6. Medicare processes the GP's bulk-bill claim against MBS item 965 (GP) or 392 (prescribed medical practitioner) — no patient form to lodge.
7. The GPCCMP can be reviewed every 3 months if clinically relevant (MBS item 967 or 393); a new plan can be prepared every 12 months.
8. Patients with a GPMP in place before 1 July 2025 may continue to access allied health services under their existing plan until 30 June 2027 without any action required.
