How to apply

  1. Confirm eligibility: be a permanent Queensland resident aged 65+ working under 35 hours/week, OR aged 60-64 working under 35 hours/week while holding a Commonwealth Pensioner Concession Card, Health Care Card, or DVA card (Seniors Business Discount Card is available to anyone aged 60+ with no work restriction)
  2. Gather one piece of identifying information: Centrelink customer reference number, DVA file number, Queensland driver licence number, or Queensland proof of age/photo ID card number
  3. Apply online via the Queensland Government Seniors Card portal at qld.gov.au/seniorscard — applications can be lodged up to two months before you turn 60
  4. Alternatively, call 13 QGOV (13 74 68) to apply by phone
  5. Or download and print the PDF application form from qld.gov.au, complete it in block letters, attach proof of age, and post to Card Services, Reply Paid 10817, Brisbane Adelaide Street QLD 4000, or scan and email to cardservices@smartservice.qld.gov.au
  6. Or visit a Queensland Government Service Centre in Brisbane, Cairns, or Maroochydore, or any Queensland Government Service Counter
  7. Wait up to 4 weeks to receive your card by mail upon approval
