How to apply

1. Confirm eligibility: the program is for people aged 25–74 with a cervix; the NCSR auto-enrols participants using Medicare records.
2. Choose your action: opt out of screening reminders, defer next screen, resume participation, update address/contact details, nominate a healthcare provider, or update communication preferences.
3. Select your preferred channel: Participant Portal at ncsr.gov.au, an online webform (e.g. 'Cervical Screening — Request to Opt Out' at ncsr.gov.au/contact/forms/), a downloadable PDF form, or by calling 1800 627 701.
4. If using the Participant Portal: log in via myGov (Medicare linked) and select the relevant action from the dashboard.
5. If using a webform: provide your Medicare number or NCSR ID, full name (as on Medicare), date of birth, and address; for opt-out, select your reason and acknowledge the consequences.
6. If calling 1800 627 701 (Mon–Fri 8am–6pm local time): confirm your identity with Medicare details and request the change verbally.
7. To update a correspondence address separately from Medicare: use the Participant Portal or call 1800 627 701 (note: primary address changes must go via Medicare on 132 011 or servicesaustralia.gov.au).
8. After submission: changes take effect within the NCSR; you can reverse an opt-out at any time using the same channels.
