How to apply

1. Report the motor vehicle crash to WA Police and obtain a police report reference number.
2. Visit crashreport.com.au and complete the online Crash Report form to formally report your injuries to ICWA — the form takes approximately 40 minutes, cannot be saved mid-way, and must be completed in one session on a laptop or desktop.
3. ICWA will send you a 'Notice of Intention to Make a Claim' form and a Medical Authority form after receiving your crash report.
4. Have your treating doctor complete the Medical Authority form and provide a medical report confirming your injuries.
5. Return the completed Notice of Intention to Make a Claim form and Medical Authority to ICWA as soon as practicable.
6. If you have lost income, ask your employer to complete the 'Employers Confirmation of Loss of Salary' form and submit it to ICWA.
7. Complete the 'Electronic Funds Transfer Application for Claimants' form to receive out-of-pocket expense reimbursements by EFT.
8. ICWA will assess your claim under WA's no-fault scheme; both fault and no-fault parties may be eligible for benefits including medical expenses and income replacement.
9. For common law damages, seek legal advice; proceedings must be commenced within 3 years under the Limitation Act 2005 (WA).
