How to apply

  1. Report the accident to police at the scene or at a police station as soon as practicable; obtain the QP reference number (Queensland Police).
  2. Identify the at-fault vehicle's registration number and use the MAIC insurer-finder at maic.qld.gov.au to identify the relevant CTP insurer.
  3. Select the correct claim form: 'Notice of Accident Claim Form (Non-Fatal Injury)' for injury claims, or the fatal injury version for fatal accident claims. Both PDF and online (Queensland CTP portal) options are available.
  4. Have your treating medical practitioner complete the Medical Certificate form to accompany your claim.
  5. Complete the Claimant Certificate form (self-declaration of injury impact).
  6. Compile supporting documents: certified proof of identity or certified photograph, and a law practice certificate if you are legally represented.
  7. Lodge the completed claim form and all supporting documents with the at-fault vehicle's CTP insurer via the secure Queensland CTP claim portal at ctpportal.maic.qld.gov.au, or send the PDF directly to the insurer.
  8. The insurer must acknowledge your claim and begin initial assessment; seek early rehabilitation support as entitled under the scheme.
  9. If your claim is disputed, contact MAIC or seek legal advice; a 3-year limitation period applies to commence legal proceedings.
