How to apply

  1. Immediately secure the incident site — do not disturb it except to help injured persons or make the area safe
  2. Identify whether the incident is 'notifiable': death of a person, serious injury or illness, or a dangerous incident as defined under your state or territory WHS Act
  3. Immediately notify your state or territory WHS regulator by phone (SafeWork NSW: 13 10 50; WorkSafe VIC: 13 23 60; WHS QLD: 1300 362 128; SafeWork SA: 1300 365 255; WorkSafe WA: 1300 307 877; NT WorkSafe: 1800 019 115; WorkSafe TAS: 1300 366 322; Comcare: 1300 366 979)
  4. Preserve the incident site and do not disturb it until a WHS inspector arrives or grants permission — serious penalties apply for disturbing the site without authorisation
  5. Complete and submit a written notification to the regulator if requested — most regulators also offer online notification portals (e.g. SafeWork NSW online form at safework.nsw.gov.au)
  6. Record full details of the incident including date, time, location, injured parties, nature of injury, and actions taken
  7. Investigate the incident and implement corrective actions to prevent recurrence
  8. Keep records of the notification and all subsequent communications with the regulator
