How to apply

  1. Confirm eligibility: you must be a current or former ADF member (including Reserves and cadets) who served on or after 1 July 2004, or a dependant of a deceased member.
  2. Gather supporting documents: ADF PMKeyS full service record, ADF medical records relevant to your condition, and your most recent ADF or SVA payslip.
  3. Preferred online path — log in to MyService at my.dva.gov.au and submit your claim digitally; you will need to verify your identity through myGovID.
  4. Paper path — download Form D2051 from the DVA forms page at dva.gov.au/about-us/dva-forms/claim-for-liability-andor-reassessment-of-compensation-under-mrca and complete one form per condition claimed.
  5. For each condition, attach a completed D2049 Injury or Disease Details Sheet (three copies are included at the back of D2051); your treating doctor must complete the relevant sections.
  6. Submit the completed form and all supporting documents to DVA — post to the address on the form, deliver to a DVA office, or lodge through a Vietnam Veterans or RSL advocate.
  7. Note: from 31 March 2025, DVA will not process incomplete claims, so ensure all attachments are included before lodging.
  8. DVA will acknowledge receipt and assign a case officer; initial liability decision is made under MRCA and you are notified in writing.
  9. If accepted, DVA will assess compensation entitlements (incapacity payments, permanent impairment, rehabilitation) and contact you about next steps.
