How to apply

1. Confirm whether Form 80 is required: it is not mandatory for all visa applications but is commonly requested for permanent visas, partner visas, and cases involving criminal history, higher-risk travel documents, or inconsistent personal information.
2. Download Form 80 (PDF) from the Department of Home Affairs forms listing at immi.homeaffairs.gov.au/form-listing/forms/80.pdf.
3. Complete the form in English — it must be filled out by any applicant aged 16 years or over as requested by the processing office.
4. Provide accurate details in all sections: personal identity (all names used), nationalities and citizenships, residential addresses for the past 10 years, employment history, education, travel history (all countries visited or lived in for 12+ months), military or law enforcement service, and any criminal history or charges.
5. Use Part T (additional information) if you need more space for any section.
6. Sign and date the declaration confirming all information is true and correct.
7. Upload the completed and signed Form 80 to your ImmiAccount as an attachment to your visa application, or provide it as directed by your case officer.
8. If you have a migration agent, they may upload the form on your behalf.
9. Retain a copy of the completed form for your records in case the Department requests clarification or additional information.
