How to apply

1. Receive a request from your Department of Home Affairs case officer to complete Form 1221, or identify that your visa application type requires it as a standard document.
2. Download Form 1221 from the Department of Home Affairs website (immi.homeaffairs.gov.au/form-listing/forms/1221.pdf).
3. Complete the form in English using block letters and a pen, or use the fillable PDF version.
4. Complete Part A — primary applicant details: full name, date of birth, all citizenships held, and current address.
5. Provide identity document details including all passports held (current and expired), any other identity documents, and any alternative names or aliases used.
6. Disclose your visa and travel history: all countries visited or resided in and all previous Australian visa applications and outcomes.
7. Provide details of any family relationships to other visa applicants or Australian citizens/permanent residents as required.
8. Sign and date the declaration confirming that all information provided is true and correct.
9. Upload the completed form as an attachment in ImmiAccount or submit it as directed by your case officer; retain a copy for your records.
