How to apply
- The former employee requests an Employment Separation Certificate from their employer — Services Australia or Centrelink may also request it directly from the employer when processing a new claim.
- Employer downloads the SU001 form from servicesaustralia.gov.au/su001 or completes it online through the Business Hub at business.gov.au.
- Employer completes Part A: business details including ABN, business name, address, and contact information.
- Employer completes Part B: employee details including full name, address, and dates of employment (start and end).
- Employer completes Part C: reason for separation (e.g. resignation, redundancy, dismissal, end of contract, stood down), gross weekly wage at separation, and details of final payment including any leave entitlements paid out, redundancy payments, or amounts still owed.
- Employer signs and dates the certificate, then provides it to the former employee or directly to Services Australia if requested.
- Employer must complete and provide the certificate within 14 days of receiving the request.
- Former employee submits the certificate to Services Australia as part of their income support claim (e.g. JobSeeker Payment, Youth Allowance) — it can be lodged online via myGov, in person at a service centre, or by post.
