How to apply

  1. Notify your employer of your workplace injury and lodge a workers compensation claim with ReturnToWorkSA or your employer's insurer.
  2. If you are likely to be away from work for more than four weeks, your mobile claims manager or Return to Work Coordinator will initiate the planning process.
  3. Download the 'Recovery and return to work plan' PDF from the RTWSA forms page at rtwsa.com/publications/forms.
  4. Complete the plan collaboratively with your employer and claims manager, documenting your recovery goals, required treatment, and support services.
  5. If a phased reintegration is needed, also complete the 'Graduated return to work schedule' form (available from the same forms page) and obtain your treating doctor's approval before commencing.
  6. Sign and share the finalised plan with all parties — injured worker, employer, treating doctor, and claims manager.
  7. Review the plan regularly and update it as your recovery progresses until you reach your return to work goal.
