How to apply

1. Confirm eligibility: you must be aged 5 or older, an Australian citizen or permanent resident, and have permanent and severe incontinence confirmed by a registered health professional.
2. Download the CAPS Application Guidelines and Application Form from health.gov.au or request it by calling 1800 239 309 (Services Australia CAPS team, Mon–Fri 8:30am–5pm AEST).
3. Have your treating registered health professional complete Section 3 of the form, confirming your diagnosis and continence management plan.
4. Complete your sections of the form including personal details, Medicare/Centrelink details, and bank account for payment.
5. Submit the completed form to Services Australia by post or, if your Medicare online account is linked to myGov, apply online via myGov or the myGov app.
6. Services Australia assesses your application and, if approved, enrolls you in CAPS.
7. Receive your annual payment in July (one annual payment) or in two bi-annual payments in July and January of each financial year.
8. Renew your enrollment annually — you will need ongoing confirmation of eligibility from your health professional if your circumstances change.
