Support at Home Service Agreement — Sign with Your Provider
Your service agreement is the contract between you and your Support at Home provider. It sets out the services you will receive and their prices, your care costs and contributions, your rights and responsibilities (and your provider's), and the complaints process. It replaced the old Home Care Agreement when Support at Home began on 1 November 2025.
