Aged Care Worker — Mandatory Statutory Declaration (international residence)
Form number: Stat Dec — Aged Care international residence
On top of the Aged Care Worker Screening Check, anyone working in residential aged care or home care who has lived overseas for 12 months or more in the past 10 years must complete a statutory declaration plus provide an international police check from each country lived in.
