Child passport — special circumstances (Form B-11)
Form number: B-11
Use Form B-11 when not all people with parental responsibility can give consent for a child's passport — e.g. one parent is deceased, overseas and uncontactable, abusive, or there is a court order. DFAT examines each case individually and may require court documentation, statutory declarations, or police reports.
