Assessment Outcome Letter (Notice of Decision) — Activate Your Aged Care Funding
After your assessment, the assessment organisation sends you a Notice of Decision letter listing the services you have been approved for (e.g. a Support at Home ongoing funding classification, or permanent residential aged care), together with your support plan. This letter is your key to services — read it carefully and act on it.
