Aged Care Reassessment — Request Higher Level of Care
If your needs go up — after a hospital stay, fall, dementia diagnosis or your carer becomes unwell — you can ask for a support plan review or reassessment to move to a higher Support at Home classification, from CHSP to Support at Home, or from home support to residential care.
