How to apply

1. Determine whether the new employee is eligible to choose a super fund (most employees are; some defined-benefit schemes and certain enterprise agreements are exempt).
2. Download the current Superannuation standard choice form (NAT 13080) from ato.gov.au or use your payroll software's built-in version.
3. Before giving the form to the employee, complete the employer section — fill in your default super fund's name, ABN, USI (unique superannuation identifier) and contact details.
4. Provide the completed form to the employee within 28 days of their commencement date.
5. Allow the employee to complete the employee section nominating their chosen fund and return it to you — they do not need to return it to the ATO.
6. Check the employee's nominated fund is a complying super fund (verify via Super Fund Lookup at superfundlookup.gov.au).
7. Begin making super contributions to the nominated fund — or, if no form is returned, request the employee's stapled super fund from the ATO before defaulting to your default fund.
8. Keep a copy of the completed form for at least five years — you do not send it to the ATO.
