In the Northern Territory, your advance care wishes are recorded in an Advance Personal Plan under the Advance Personal Planning Act 2013 (NT). The Plan combines health-care directives, an advance care statement, an advance consent decision and the appointment of a decision-maker; advance personal planning replaces enduring power of attorney in the NT.
