Application for Cremation — QLD

In Queensland the Cremations Act 2003 requires Form 1 — Application for a permission to cremate. The permission itself is issued by an independent doctor (a doctor who did not sign the cause of death certificate or the cremation risk certificate) on Form 4, or by a coroner where the death is reportable. Important: an objection from a spouse, adult child, parent or personal representative stops the cremation, unless the person who died left signed instructions to be cremated.
