In Queensland, the Cremations Act 2003 (QLD) requires Form 1A — Application for Cremation — signed by the senior next-of-kin, plus a Form 2 Cremation Permit signed by a medical referee. If the Coroner is involved, no cremation until the Coroner issues approval.
